First Visit
Upon entering our office you will be greeted cordially and asked to complete
a few health information forms. These forms give us vital information regarding
your past and present health concerns so that we can give you the best
care possible. It only takes a few minutes to complete. If you would prefer,
the patient
forms can be downloaded from our website so that you can complete them prior
to entering our office. For those patients who have suffered
auto injuries please bring a copy of the accident report if you have one,
and any auto insurance information you have. Please tell your adjustor
you are seeking medical attention. Other health insurance information
is normally given to the office during this visit so it can be determined
what type of coverage you have.
Any insurance information we have received will be reviewed with you so that
you will know what your insurance company says your financial obligation will be
for the treatment you are receiving. You will also have ample opportunity to ask
questions to your satisfaction.
After speaking with you and receiving a detailed health history,
we will ask you specific questions regarding your current health
issue. Samples of questions to think about would be what
type of pain do you have (sharp, achy, burning), where is the
pain located; are there any movements or activities that increase
the pain, or for that matter, make the pain go away; what type
of treatment have you used so far for this condition, etc.
We then take time to review the results of all test results and
design a treatment plan best suited for YOU. In most cases your
treatment can begin immediately!
2nd Visit
The purpose of the second visit is to explain, in detail, the findings of all
tests performed on the first visit.
Any insurance information we have received will be reviewed
with you so that you will know what your insurance company
says your financial obligation will be for the treatment you
are receiving. You will also have ample opportunity to ask
questions to your satisfaction.
Once all of these topics are explained, we have reserved
time to treat you on the second visit so that we can start
making progress immediately.
Our job is to report to you the type of treatment we feel
is the best for your condition, it is ultimately up to you
to determine to what degree you want treatment and how fast
you want results. We feel confident that we can customize your
care to give you the best opportunity to achieve the results
you desire.
